We are looking for an experienced, dynamic RN Case Manager to join our team.
Position Summary
As a Case Manager, you will be responsible for the coordination of care with the patient, family, and other members of the team. Our Case Managers work as RN/LPN teams so we need someone with great supervisory and organization skills.

Knowledge, Skills and Abilities
- Must be comfortable with driving.
- Must be comfortable using EMR
- Must have an energetic and positive demeanor
- Must be able to remain calm under patient pressure.
Education & Experience
- Must be currently licensed as an RN in Georgia
- Previous experience in hospice, preferably working with RN/LPN teams
