Position Summary
The Transport Driver/CNA is responsible for providing safe, timely transportation for residents to and from medical appointments, dialysis, hospital visits, and other approved destinations while ensuring resident comfort and dignity. When not transporting residents, this position functions as a Certified Nursing Assistant, providing quality direct resident care in accordance with individualized care plans.
Essential ResponsibilitiesTransportation Duties
- Safely transport residents using facility vehicles.
- Assist residents entering and exiting vehicles, including wheelchair securement.
- Ensure residents arrive safely and on time for appointments.
- Complete transportation logs and required documentation.
- Perform vehicle safety inspections and report maintenance concerns.
- Maintain vehicle cleanliness and compliance with safety standards.
- Communicate appointment updates with nursing staff and transportation coordinators.
Certified Nursing Assistant Duties
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
- Obtain and record vital signs as assigned.
- Assist residents with mobility, transfers, and ambulation using proper lifting techniques.
- Observe and report changes in resident condition to licensed nursing staff.
- Maintain accurate documentation of care provided.
- Promote resident dignity, independence, and quality of life.
- Follow infection prevention and safety procedures.
Qualifications
- Current Florida Certified Nursing Assistant (CNA) certification required.
- Valid Florida Driver's License with a clean driving record required.
- High school diploma or GED preferred.
- CPR certification preferred (or willingness to obtain).
- Experience in skilled nursing, rehabilitation, assisted living, or long-term care preferred.
- Ability to safely operate wheelchair-accessible transportation vehicles.
- Excellent customer service and communication skills.
- Ability to lift, push, pull, and assist residents in accordance with facility policy.

Regulatory Requirements
Employment is contingent upon successful completion of all pre-employment requirements, including:
- Level 2 background screening in accordance with Florida AHCA requirements.
- Drug screening.
- Verification of CNA certification and employment eligibility.
- Compliance with all state and federal regulations applicable to long-term care employees.
