Occupational Therapist (OT) – Home Health (Per Diem)
We are seeking a compassionate and motivated Occupational Therapist (OT) to join our home health team on a per diem basis. This role offers flexibility and the opportunity to make a meaningful impact by helping patients regain independence and improve their quality of life in the comfort of their own homes.
Responsibilities:
- Evaluate and treat patients in their homes to improve functional independence
- Develop and implement individualized treatment plans based on patient needs
- Address ADLs, functional mobility, upper extremity strength, coordination, and safety
- Provide patient and caregiver education for carryover and long-term success
- Collaborate with physicians, physical therapists, and interdisciplinary team members
- Complete timely and accurate documentation in compliance with Medicare Part B / home health guidelines
Qualifications:
- Active Occupational Therapist license in the state of New Jersey
- Minimum 1 year of clinical experience (home health experience preferred but not required)
- Strong clinical reasoning and communication skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation required
