RN- Case Manager for AFC and GAFC (Adult and Group Foster care)

We are seeking a dedicated Registered Nurse (RN) with strong leadership experience to support our growing team. This role is ideal for an RN who is passionate about quality care, teamwork, and guiding staff toward excellence.

Essential Duties & Responsibilities

• Provides leadership, clinical oversight, and guidance to GAFC/AFC staff to ensure high‐quality care, strong teamwork, and adherence to program standards.
• Performs participant Assessment, Admission, 2‐week, 4‐week, and 60‐day visits, with documentation completed within 2 days of target dates.
• Conducts post‐hospital visits, including a 24‐hour RN visit after discharge, followed by 2‐week, 4‐week, and 60‐day assessments.
• Documents participant visits using outcome‐oriented Nursing Progress Notes, highlighting functional status, service needs, mental status, medication changes, health updates, home environment, satisfaction with services, and compliance with the care plan.
• Creates an Individualized Service Plan/Med Sheet based on the MDS and Physician Summary in collaboration with the RSC.
• Ensures nursing notes are written as needed to address significant changes and health‐teaching.
• Assists in the delivery of GAFC /AFC program services as needed.
• Trains GAFC /AFC support staff, including in‐service education.
• Reviews and implements physician orders.
• Completes all required reports in a timely manner.
• Performs other duties as assigned.

Education & Experience Requirements

• Computer skills, including MS Word, Excel, Outlook, and experience with EMR systems.
• Ability to work under pressure, meet deadlines, and maintain a positive, professional attitude.
• Able to work independently and complete assignments within established guidelines.
• Associate Degree in Nursing required; Bachelor’s degree preferred.
• Current MA RN license and CPR certification required.
