• Shift: Days (40 hours/week) with rotating weekend coverage
• Salary Range: $58240.00– $77168.00 per year
• Bonus: $7,500 Sign on Bonus

Requirements

• Bachelor of Science degree in Human Nutrition and Dietetics (required).
• Registered Dietitian (RD) or Registration Eligible (required).
• Certified Dietitian/Nutritionist (CDN) in New York State (preferred).
• 1 year of acute care or long-term care experience (preferred).

Responsibilities

• Conduct nutrition assessments and identify patients/residents at risk for malnutrition.
• Perform Nutrition Focused Physical Exams (NFPE) to assess malnutrition levels.
• Develop individualized diet plans and monitor progress/effectiveness of interventions.
• Provide nutrition education and counseling to patients/residents, families, and staff.
• Collaborate with physicians, nurses, and care teams to integrate nutrition into overall care plans.
• Maintain accurate documentation in compliance with facility and regulatory standards.
