Case Management Nurse

Job Title: RN Care Manager – Direct Hire (Days)

• Shift: Days (9:00 AM – 5:00 PM)
• Weekend Requirement: Minimum 2 shifts per month
• Guaranteed Hours: 24 hours/week
• Salary Range: $60/hr – $76/hr

What We’re Looking For

• BSN required
• Active NYS RN license
• PRI certification (or able to obtain within 3 months)
• Strong computer and EMR skills
• 3 years in ED, critical care, or med-surg
• Prior case management experience
• Screening certification
• Care Management certification (CMC, CCM, etc.)
• Working knowledge of payer/regulatory guidelines and Milliman/InterQual criteria

What You’ll Do

• Complete comprehensive initial assessments and ongoing reassessments throughout hospitalization
• Coordinate multidisciplinary care across hospital levels
• Lead discharge planning, including patient/caregiver education, aftercare arrangements, payer communication, and high-utilizer interventions
• Perform Utilization Review (UR) duties including payer reviews, admission reviews, and clinical documentation using Milliman/InterQual criteria
