Job Title: RN Care Manager – Direct Hire (Days)

- Weekend Requirement: Minimum 2 shifts per month
- Guaranteed Hours: 24 hours/week
- Salary Range: $60/hr – $76/hr

What We’re Looking For
- BSN required
- Active NYS RN license
- PRI certification (or able to obtain within 3 months)
- Strong computer and EMR skills
- 3 years in ED, critical care, or med-surg
- Prior case management experience
- Screening certification
- Care Management certification (CMC, CCM, etc.)
- Working knowledge of payer/regulatory guidelines and Milliman/InterQual criteria
What You’ll Do
- Complete comprehensive initial assessments and ongoing reassessments throughout hospitalization
- Coordinate multidisciplinary care across hospital levels
- Lead discharge planning, including patient/caregiver education, aftercare arrangements, payer communication, and high-utilizer interventions
- Perform Utilization Review (UR) duties including payer reviews, admission reviews, and clinical documentation using Milliman/InterQual criteria
