Role Description
We are seeking a full-time LVN/RN Kaiser Case Manager to join our team in Los Gatos, CA. This is an on-site role that involves coordinating and managing patient care plans, collaborating with interdisciplinary teams, and ensuring continuity of care for residents. The Case Manager will assess patient needs, develop individualized care plans, and monitor their implementation. This role also includes maintaining accurate documentation, communicating with families, and supporting a patient-centered approach to healthcare delivery.
Qualifications
- Active LVN or RN license in good standing with the State of California
- Experience in case management, care planning, and patient assessments
- Strong organizational and time-management skills
- Excellent communication, collaboration, and problem-solving abilities
- Knowledge of Medicare guidelines, skilled nursing facility regulations, and healthcare documentation
- Proficiency in electronic medical records (EMR) systems
- Compassionate approach toward patient care and a commitment to improving patient outcomes
- Prior experience in a skilled nursing or rehabilitation setting is highly desirable
