Nurse Practitioner – Hospice Care

Location: Houston, TX (home visits)
Under the general supervision of a designated physician and in accordance with standardized procedures, the Nurse Practitioner (NP) provides compassionate hospice and palliative care to patients in their homes. This includes performing health assessments, managing common acute and chronic stable conditions, and collaborating with the care team to ensure continuity of care.
Essential Responsibilities
- Provide primary care to hospice patients, including health histories, physical exams, lab tests, prescriptions, and referrals within current clinical guidelines.
- Perform accurate and timely documentation in the Electronic Health Record (EHR) within 72 hours of each visit.
- Focus on patients with chronic, unstable, or multi-system problems, while also addressing acute simple illnesses and preventive care.
- Educate and counsel patients and families on health, safety, lifestyle, and end-of-life care planning.
- Collaborate with physicians and the hospice team to evaluate care needs and adjust treatment plans.
- Communicate effectively and compassionately with patients, families, and healthcare professionals.
- Participate in the development of standards of care and comply with all hospice, Medicaid, and Medicare guidelines.
Qualifications
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State license as a Registered Nurse (RN) and Advanced Practice Nurse (APN).
- Current Driver’s License and reliable transportation with valid auto liability insurance.
- Experience: Minimum 2 years as an APN preferred; hospice or home health experience strongly desired.
- Skills: Strong nursing knowledge, clinical decision-making, and excellent interpersonal communication.
Work Environment & Requirements
- Patient home visits in a variety of settings.
- Possible exposure to blood, body fluids, infectious diseases, and unpleasant weather.
- Flexible schedule required; PRN emergency call availability.
- Physical ability to stand, walk, and lift up to 50 lbs; ability to move patients.
- Work may involve stressful conditions and quick decision-making.
