Responsibilities
Patient Care and Assessment:
- Perform thorough patient assessments to create and execute individualized care plans.
- Administer medications, wound care, and specialized treatments as prescribed.
Monitoring and Education:
- Monitor patient conditions and provide education to patients and families.
- Offer guidance on healthy practices, medication management, and disease prevention.
Collaboration and Documentation:
- Coordinate care with the interdisciplinary team.
- Document patient progress accurately and timely in the EMR system.
Emotional Support:
- Provide empathy and support to patients and families.
- Advocate for patients’ needs and comfort.
Adaptability and Problem-Solving:
- Adjust care plans as needed.
- Handle unexpected situations or emergencies effectively.
Requirements
- Active RN license in the state of Texas.
- Home health experience preferred.
- Valid driver’s license required.
- Kinnser/WellSky EMR experience highly desired.
Job Types: Contract, PRN
Pay: $55.00 - $110.00 per hour
