What You’ll Do:
- Evaluate patients and develop individualized speech therapy treatment plans
- Provide in-home speech therapy services
- Help patients improve speech, language, communication, cognitive, and swallowing abilities
- Educate patients and caregivers on exercises, communication strategies, and safety techniques
- Monitor patient progress and adjust care plans as needed
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Complete accurate and timely documentation
What We’re Looking For:
- Active Texas Speech-Language Pathologist (SLP) license (required)
- Home health experience preferred
- Strong clinical judgment and patient care skills
- Ability to work independently in a home setting
- Excellent communication and documentation skills
- Reliable transportation & valid driver’s license
- Kinnser/WellSky EMR experience (preferred)
