What You’ll Do:
- Provide skilled in-home nursing visits based on the plan of care
- Perform vital signs, medication administration, wound care, and catheter care
- Observe and report changes in patient condition to the RN Case Manager
- Educate patients and caregivers on medications, safety, and disease management
- Complete accurate and timely documentation
- Communicate effectively with the care team
What We’re Looking For:
- Active Texas LVN license (required)
- Home health experience preferred
- Strong clinical and communication skills
- Ability to follow care plans and adapt to patient needs
- Reliable transportation & valid driver’s license
- Kinnser/WellSky EMR experience (preferred)
