Vantage Healthcare is looking for a Full Time Nurse Practitioner or Physician Assistant to preform primary care visits in Skilled Nursing and Assisted Living Facilities in Concord, NH and surrounding areas.
Responsibilities: Provide primary care visits, med reconciliations, and annual wellness visits 4 or 5 days a week .

Our patients will be treated in the comfort of their own residence, assisted living, memory care, or skilled nursing facility for any problems, conditions, and/or injuries for which they would normally go to a primary-care physician. We also offer an online patient portal to keep patients and healthcare proxy up to date on medications, conditions, and upcoming appointments
Responsibilities
- Examine patients and their medical records
- Order and study diagnostic tests (blood tests, X-rays etc.)
- Diagnose health conditions and illnesses
- Propose treatments for chronic and infectious diseases
- Prescribe medications
- Maintain accurate records and schedules
- Inform patients about their health conditions and prescribed medications
- Educate patients and their family members on diseases, epidemics and prevention
- Oversee patients’ overall health
- Promote good health practices
- Advance professional knowledge and conduct research
