Responsibilities
- Conduct comprehensive patient assessments, including histories, physical examinations, and health screenings
- Diagnose and treat acute and chronic illnesses in accordance with clinical guidelines and scope of practice
- Manage chronic diseases such as diabetes, hypertension, and cardiovascular conditions to optimize patient outcomes
- Develop and implement individualized preventive care and health maintenance plans
- Order, interpret, and follow up on diagnostic tests, laboratory work, and referrals
- Prescribe medications in compliance with federal and state regulations, including DEA requirements
- Document patient encounters accurately and thoroughly in the electronic health record (EHR)
- Collaborate with physicians and interdisciplinary teams to coordinate patient care and treatment plans
- Educate patients and families on disease prevention, medication management, and healthy lifestyle choices
- Maintain compliance with clinical policies, quality standards, and regulatory requirements
- Master's or Doctorate (MSN or DNP) from an accredited Nurse Practitioner program
- Active Ohio APRN-CNP license
- National Nurse Practitioner board certification (AANP or ANCC)
- Active DEA registration
- Demonstrated expertise in primary care patient assessment, diagnosis, and treatment
- Proficiency in chronic disease management and preventive care
- Strong skills in EHR documentation and clinical record-keeping
- Preferred: Experience as a primary care or family practice Nurse Practitioner
- Preferred: Experience in care coordination and patient education
- Preferred: Ability to thrive in a collaborative, team-based care environment
