Key Responsibilities
- Build and maintain strong relationships with medical professionals, hospitals, and community healthcare partners
- Represent and promote the organization’s mission, philosophy, and services both within hospitals and throughout the community
- Attend facility meetings, calls, and educational in-services
- Maintain accurate and up-to-date referral tracking logs
- Provide timely end-of-day reports to department heads
Qualifications
- Valid driver’s license required; travel is a key component of this role
- RN, LPN, or LSW preferred, but not required
- Ability to manage multiple residents and referrals in a fast-paced environment
