Community Habilitation Specialist - Per Diem

The Community Habilitation Specialist is responsible for coordinating and assisting individuals and families to achieve specific goals by providing respite, residential habilitation services, skill building, in and/or parent assistance services. Per Diem position - (Per Diem$20/hr.)

Primary Duties & Essential Functions

• Maintain knowledge of community-based resources including, but not limited to, age-appropriate recreational activities.
• Provide direct care and services to individuals and families.
• Assist individuals with goal orientated activities based on agreed upon services plan.
• Attend and participate in supervision, staff development opportunities and agency/community committees.
• Communicate and interact with individuals and families appropriately and effectively.
• Explains and promotes Sanctuary to prospective employees, volunteers and the community.
• Create personal schedule based on individuals served utilizing appropriate time management techniques.
• Complete written service summaries as required.
• Complete assigned paperwork, including intake assessments and discharge summaries in a timely manner.
• As this list of duties and responsibilities are not intended to be all-inclusive, other tasks and responsibilities may be assigned from time to time as business needs require.
  
  

Required Education, Knowledge And Skills

• Valid NYS Driver’s License with clean driving record.
• High School diploma or GED required.
• Ability to manage time and workload effectively.
• Problem solving skills.
• Ability to communicate effectively, orally and in writing.
• An equivalent combination of education, experience, knowledge, skills, abilities.
