Description:Are you looking to put your Values Into Action? To do work that truly matters?Values Into Action- NJ is hiring a Support Coordinator to join our team. This position requires the Support Coordinator to be fluent in Spanish and English languages (speaking, reading, and writing). As a Support Coordinator, you will assist individuals with intellectual and/or developmental disabilities to coordinate both paid and non-paid services and supports that will aid them in living a fulfilling life within their community. This includes working with the service user and their team to create an individualized service plan that clearly includes the person’s aspirations and goals; and on-going monitoring of services and completion of service documentation to ensure that the individuals’ needs are being addressed and regulatory standards met.This position offers flexibility with some tasks that can be completed remotely. However, it also requires regular engagement in the community and travel within the region to meet the needs of those supported and required by our funders. The ideal candidate will live in or around the New Brunswick/Edison/Piscataway areas of New Jersey. Frequent travel throughout the state of New Jersey is expected in this role.
