Overview:
The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Your role will involve following the plan of care, providing treatments, and collaborating with the team to ensure the best possible care for our patients.
Duties & Responsibilities:
- Provide direct patient care as defined in the State's Nurse Practice Act
- Implement the plan of care initiated by the registered nurse
- Accurately and timely document patient care consistent with the plan of care
- Assess and provide patient and family education pertinent to diagnosis and plan of care
- Participate in coordination of home health services and report needs to the registered nurse
- Use equipment and supplies effectively and efficiently
- Participate in personal and professional growth and development
- Perform other duties as assigned by the registered nurse

Job Requirements:
- Graduate of an accredited practical vocational nursing program
- Three years of nursing experience preferred, with hospice/home health or medical/surgical experience being a plus
- Current license as a licensed practical nurse or licensed vocational nurse in the state
- Compliance with accepted professional standards and practice
- Good verbal and written communication skills
- Strong organization skills
- Must be a licensed driver with a reliable insured automobile
- Possess and maintain current CPR certification
