What are the responsibilities and job description for the Admission Nurse position at VALOR HOSPICE?
Primary functions are to perform admissions and administer nursing care for patients of all ages in their place of
residence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume the
responsibility for coordination of care.
Essential Functions:
Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, and
environmental needs.
Develop and document the plan of care in correlation with Assessment
Provides nursing care utilizing infection control measures (OSHA)
Work with Interdisciplinary Group/Team to ensure patient/family needs are met.
Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
Timely submission of all required paperwork.
Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and
maintaining or improving competency. Participates in the Hospice’s QAPI program.
Promotes the Hospice’s philosophy and administrative policies.
Meets productivity standards.
Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
Provides effective communication to patients, their family members, team members, and other health care
professionals.
Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current State license as a Registered Nurse; Current driver’s license.
Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.
Good interpersonal skills.
