We are seeking a compassionate, autonomous, and clinically strong Certified Nurse Practitioner (CNP/APRN) to serve as a key leader in caring for residents across nursing facilities in the Dayton and surrounding areas.

As a Nurse Practitioner with Valor Clinical Partners, you will play a vital role in managing care for nursing home residents and coordinating high-quality, cost-effective interventions. You will serve as a trusted clinical leader and advocate for members while collaborating with facility teams and interdisciplinary partners.

What You’ll Do

Primary Responsibilities

  • Deliver comprehensive primary and episodic care to residents in long-term care and skilled nursing settings
  • Conduct ongoing clinical assessments and manage care based on health risk indicators and changing medical needs
  • Evaluate residents following hospitalizations and transitions of care to support recovery and reduce readmissions
  • Diagnose, treat, and manage acute and chronic conditions with a strong focus on early intervention and treatment in place
  • Perform physical examinations, prescribe medications and interventions, and monitor clinical outcomes
  • Triages care needs utilizing risk stratification and clinical assessment tools
  • Collaborate with physicians, specialists, behavioral health providers, nursing staff, and facility leadership to coordinate care
  • Educate residents and family members regarding disease management, treatment plans, medications, and goals of care
  • Provide clinical guidance and education to nursing facility staff to support high-quality resident outcomes
  • Serve as a proactive advocate for residents while promoting continuity, communication, and patient-centered care
  • Maintain timely and accurate clinical documentation and compliance standards
  • Participate in interdisciplinary care planning and quality improvement initiatives

What a Typical Day Looks Like

Your day starts with flexibility and purpose. You’ll typically visit 1–2 skilled nursing or long-term care facilities, seeing residents, addressing acute needs, following up after hospital discharges, collaborating with nursing staff, and helping prevent unnecessary emergency department visits or hospitalizations.

You will have the opportunity to build meaningful relationships with residents and become a valued clinical partner to facility teams—while maintaining a schedule that supports your life outside of work.

Qualifications

Required

  • Active, unrestricted license to practice as a Certified Nurse Practitioner (CNP/APRN) in the State of Ohio
  • Current prescriptive authority and DEA licensure (or ability to obtain)
  • Minimum 3 years of clinical healthcare experience
  • Strong assessment, diagnostic, and care coordination skills
  • Ability to work independently while collaborating within an interdisciplinary care model
  • Excellent communication and relationship-building abilities
  • Must have a valid drivers license

Preferred

  • Experience in long-term care, geriatrics, post-acute care, skilled nursing, managed care, primary care, hospital medicine, or institutional care settings
  • Experience managing medically complex or high-risk populations

Additional Requirements

Candidates must not have had a professional license suspended in any state and must not be excluded or precluded from participation in Medicare or Medicaid programs.

Salary: $115,000 annually
