Role Description
The Case Manager will play a vital role in supporting clients on their recovery journey by coordinating care, assessing individual needs, creating comprehensive treatment plans, and facilitating Groups. Daily responsibilities include maintaining documentation, conducting client assessments, engaging with therapy teams, facilitating communication between caregivers, and connecting clients to community resources when needed. This is a full-time, on-site role at our Brookhaven, GA location.
Qualifications
- Experience in case management, individualized care planning, and coordination of services
- Strong interpersonal, communication, and active listening skills to effectively collaborate with clients, families, and multidisciplinary teams
- Familiarity with behavioral health concepts, trauma-informed care, motivational interviewing, and community resources
- Proficiency in documentation, record-keeping, and organizational tools
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field; Master's degree preferred
- State licensure or eligibility for licensure as a clinical professional (e.g., LCSW, LPC, LMFT) preferred
- Ability to work independently and maintain ethical standards in a clinical environment
