This is a wonderful working environment with an experienced staff, strong leadership support, and a unique care model focused on behavioral health, medication management, and social cueing.
Position Summary
Provides professional nursing care and leadership within the community. Coordinates care and services based on resident needs and individual preferences determined through ongoing assessment and evaluation.
This is a leadership role, and a major part of the position involves working alongside staff, coaching, mentoring, training, and helping develop the caregiving team. The nurse in this role will supervise direct caregivers while promoting a positive and resident-centered culture throughout the community.
Valleyview is a behavioral health assisted living community with a primary focus on medication management, mental health support, behavioral health services, and social cueing, unlike a traditional assisted living environment.
Schedule
- Full-time position
- Four 10-hour shifts per week
- Includes an on-call rotation for phone support only
- Weekend clinical on-call responsibilities are managed by a dedicated RN team
Essential Duties
- Initiates, updates, and manages resident-centered care plans
- Continuously evaluates resident needs with input from residents, families, physicians, and the care team
- Provides leadership, coaching, and training to caregiving staff
- Supervises, mentors, and disciplines nursing support staff as needed
- Assists in creating a positive, supportive, and accountable work culture
- Recognizes changes in resident condition and coordinates interventions appropriately
- Communicates effectively with physicians, pharmacists, staff, residents, and family members
- Ensures resident privacy and confidentiality
- Oversees medication administration and documentation according to physician orders and resident service plans
- Ensures medications are properly ordered, available, and disposed of per protocol
- Coordinates therapies and outside services as needed
- Reports concerns, incidents, and resident progress to leadership
- Documents assessments, interventions, and follow-up accurately
- Supports person-centered care and successful aging initiatives
- Participates in Life Enrichment opportunities to improve resident quality of life
- Other duties as assigned
Basic Qualifications & Experience
- Minnesota Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing
- Advanced education or certifications preferred
- Minimum of 1 year of leadership or supervisory experience preferred
- Strong communication and interpersonal skills
- Ability to coach, mentor, and lead staff effectively
- Must be able to read, write, and speak English fluently
- Must possess excellent customer service skills and professional judgment
