We’re looking for a Clinical Pharmacist to join our team at our La Junta Clinic. This role is centered on direct patient care and clinical pharmacy services, working closely with providers and care teams to optimize medication therapy, improve outcomes, and support chronic disease management.
This is an excellent opportunity for a pharmacist who enjoys practicing at the top of their license in an ambulatory care setting, with a strong focus on patient education, collaboration, and quality improvement.
Key Responsibilities
- Provide clinical pharmacy services in collaboration with providers and care teams
- Review patient medications for appropriateness, interactions, allergies, and dosing
- Deliver patient education in a clear, patient-friendly manner
- Serve as a drug information resource for medical and nursing staff
- Manage anticoagulation therapy, including:
- Patient scheduling and follow-up
- INR monitoring and interpretation
- Dose adjustments and patient communication
- Support quality improvement initiatives and participate in CQI reporting
- Maintain and monitor lab equipment and clinical programs
- Assist with development and implementation of clinical pharmacy programs
- Precept and support pharmacy students as applicable
- Utilize clinical resources and literature to support evidence-based decision making
- Ensure compliance with all policies, procedures, and regulatory requirements
Requirements
- Graduate of an accredited School of Pharmacy (PharmD required)
- Active Colorado Pharmacist license (or eligible)
- Strong clinical and administrative skills
- Experience in:
- Community health or FQHC settings (preferred)
- Ambulatory care or clinical pharmacy services
- Ability to work collaboratively with providers and interdisciplinary teams
- Strong communication and patient education skills
- Comfortable using EMR systems and pharmacy-related software
- Bilingual (Spanish/English) preferred
- Demonstrates cultural competency and ability to work with diverse populations
Location & Type of Job: This is a full-time, onsite position based in La Junta, Colorado, with travel required to support clinics across the Lower Arkansas Valley region.
