We’re looking for a compassionate Nurse Home Visitor to join our Nurse Family Partnership team in Fremont County. This role is dedicated to supporting women and families through pregnancy, birth, and early childhood — empowering them through education, encouragement, and specialized nursing care.
Key Responsibilities:
- Assess physical, emotional, social, and environmental health needs of women and their families.
- Partner with families to set realistic goals with measurable outcomes that promote healthy pregnancies and positive life-course development.
- Provide counseling and instruction on prenatal/postpartum care, infant health, nutrition, parenting, and family planning.
- Evaluate progress toward goals and revise care plans as needed.
- Build relationships that encourage independence, confidence, and problem-solving skills.
- Encourage involvement of support persons during pregnancy and early childcare.
- Connect families with community resources tailored to their needs.
- Collaborate with other healthcare and social service professionals to ensure coordinated support.
- Maintain timely and accurate documentation — recording visits within 24 hours.
- Maintain caseload, visit frequency, and program outcomes in accordance with Nurse Family Partnership protocols.
- Participate in weekly supervision, joint visits, and team activities.
- Ensure prompt follow-up with new referrals (contact within 48 hrs; home visit within 10 working days).
- Assist with outreach efforts and actively engage assigned community region.
- Complete required NFP trainings and ongoing professional development.
Licensed RN Duties:
- Perform nursing functions within scope of licensure as needed.
- Maintain credentialing and competency as an Other Licensed or Certified Health Care Practitioner (OLCP).
Qualifications & Requirements:
- Baccalaureate Degree in Nursing
- Active Colorado RN license
- Valid driver’s license and reliable transportation
- Strong ability to build rapport with diverse families and communities
- Preferred: 2 years of experience with childbearing women and infants (especially in community health)
- Bilingual skills a plus
