Valley-Wide Health Systems is looking for a Community Support Specialist who is passionate about helping others and making a meaningful impact in the community. This role provides support, care coordination, and life skills assistance to help patients navigate challenges and improve overall well-being.
Key Responsibilities
- Provide care coordination and life skills training for patients with significant mental illness individually and in group settings.
- Manage caseload for Community Support Services including the Permanent Supportive Housing Program.
- Develop, implement, and monitor individualized service plans.
- Coordinate care in clinic settings, patient homes, and community environments.
- Document patient interactions and maintain accurate clinical records.
- Ensure timely documentation (within 48 hours) and meet productivity and compliance standards.
- Collaborate with clinical teams to support patient care goals.
- Attend team meetings, individual supervision, and group trainings as scheduled.
- Demonstrate cultural competence and sensitivity in all interactions and documentation.
- Provide transportation using a company vehicle to patients in behavioral health services as necessary.
- Uphold HIPAA requirements and maintain strict confidentiality at all times.
Requirements
- Bachelor’s degree in Counseling, Psychology, Social Work or related field.
- Candidates with non-related degrees may be considered with one year of relevant experience.
Preferred Qualifications
- Valid Colorado driver’s license and ability to travel as needed.
- Ability to pass a Behavioral Health background investigation.
- Strong written and verbal communication skills.
- Proficient in Microsoft Office and general office equipment.
- Demonstrates empathy, professionalism, and cultural sensitivity.
- Excellent time management and organizational skills.
- BLS Certification (provided upon hire).
- Team-oriented with a collaborative mindset.
Location & Type of Job: This job is a full-time onsite job located in La Junta, Colorado.
