Position Summary

In addition, the position includes direct caregiving responsibilities, providing hands-on support to clients in their homes and during respite sessions.
Key Responsibilities
Program Coordination
- Coordinate and oversee monthly respite sessions, including scheduling, set-up, and wrap-up.
- Develop and implement activity plans that foster engagement, socialization, and dignity for participants.
- Communicate with families about schedules, participation, and program updates.
- Collaborate with volunteers and staff to ensure safe and meaningful experiences.
- Maintain attendance records, activity logs, and care notes.
- Assist with outreach and relationship-building with community partners to enhance programming.
- Provide direct care to clients, including assistance with activities of daily living (ADLs) such as toileting, feeding, transferring, mobility, and personal hygiene.
- Offer companionship, encouragement, and support in both the respite program and individual home care settings.
- Monitor clients' well-being, report changes in condition, and follow established care plans.
- Support infection control, safety, and organizational standards at all times.
- Assist with volunteer recruitment, training, and coordination.
- Communicate effectively with the Executive Director and care team regarding program needs, family updates, and client concerns.
- Participate in staff meetings and training as required.
- Uphold Valley VNA's mission, vision, and values.
- High school diploma or equivalent required; Caregiving experience required minimum of 5 years.
- Experience working with older adults, especially those with dementia or Alzheimer's, is preferred.
- Strong organizational skills with ability to balance multiple priorities. Computer literacy required.
- Compassionate, patient, and creative approach to engaging with clients.
- Excellent communication and teamwork skills.
- Ability to lift, transfer, and provide physical support to clients as needed.
- CPR/First Aid certification (or willingness to obtain upon hire).
- Part-time, flexible hours, with respite sessions scheduled 1st and 2nd Mondays 1p-4p, 2nd and 4th Tuesday 10a-12p, and 1st and 3rd Fridays 8a-4:30p.
- Additional part-time caregiving hours will be assigned based on client needs and employee availability.
- This role requires occasional evening or weekend shifts depending on program scheduling.
- This role may require on-call availability
