About Us
Valley Integrated UC & Wellness is a growing, physician-led multi-service practice in Northeastern Pennsylvania offering urgent care, primary care, occupational health, wellness/med spa, and telehealth services — all under one roof. Led by our board certified family medicine provider, our practice is built on the belief that patients deserve accessible, whole-person care close to home. As we continue to expand our primary care line, we're looking for a Nurse Practitioner or Physician Assistant who wants to build lasting patient relationships in a collaborative, independent practice setting.
The Opportunity
You'll manage a primary care panel, seeing patients across the lifespan for wellness visits, chronic disease management, acute concerns, and preventive care. You'll work closely with our clinical team, with the autonomy of a smaller practice and the support of an established, multi-service organization behind you.
What You'll Do
- Provide comprehensive primary care: annual physicals, chronic disease management (diabetes, hypertension, hyperlipidemia, etc.), acute visits, and preventive screenings
- Develop and manage individualized treatment plans for a dedicated patient panel
- Order, perform, and interpret diagnostic tests within scope of practice
- Prescribe medications and manage medication therapy in accordance with PA licensure
- Document thoroughly and accurately in the EMR in a timely manner
- Collaborate with physicians, front desk, and clinical support staff to ensure smooth patient flow
- Coordinate care with specialists and ancillary services as needed
- Educate patients on preventive health, chronic disease self-management, and healthy lifestyle choices
- Participate in quality improvement initiatives and practice growth as the primary care line expands
What We're Looking For
Required:
- Current, unrestricted Pennsylvania licensure as a Nurse Practitioner (CRNP) or Physician Assistant (PA-C)
- Active national certification (ANCC, AANP, or NCCPA depending on credential)
- Current DEA registration or eligibility to obtain
- Current BLS certification
- Strong clinical judgment and ability to work both independently and collaboratively
Preferred:
- 1 years of experience in primary care, family medicine, or a related outpatient setting (new grads with strong clinical rotations are welcome to apply)
- Bilingual candidates encouraged to apply

Pay: $60.00 - $80.00 per hour
