NATURE OF WORK:
With supervision, the SUD Care Coordinator functions as a member of the treatment team in a specialized extended care residential program for men and women with a primary diagnosis of Substance Use Disorder. The Care Coordinator’s duties include assessment of consumer’s needs and strengths; planning and setting treatment goals and objectives with the consumer; evaluation of progress toward these goals; advocating on behalf of the consumer; and crisis assistance/intervention.
The Care Coordinator convenes the treatment team, serves as an information source, and interfaces with external services ensuring quality of availability, continuity, flexibility, and integration of necessary resources and services.
The Care Coordinator works closely with both internal and external staff to provide an efficient and smooth continuum of care for the consumer.
ESSENTIAL DUTIES:
1) Attendance
- Maintain 40 hours per week.
- Follow Valley HealthCare System and Department’s call-off procedure policy and procedures.
2) Performance
- Assess consumer’s needs and evaluate progress.
- Keep treatment plans and reviews current for the assigned caseload.
- Initiates, coordinates and facilitates interdisciplinary treatment team meetings.
- Link consumers with necessary resources and services during and after their residential treatment
- Completes and submits documentation as required by Valley, Medicaid, Medicare, Private Insurances and any other licensing agencies. Employee ensures that documentation (such as Direct Services Progress Notes, Treatment Plans, Weekly Summaries, etc.) are completed accurately and timely and will reflect services the client(s) has received as appropriate.
- Adhere to productivity standards set forth by Supervisor
- Review consumer charts and documentation from other programs or service providers on a monthly basis.
- Staff spends time participating in group or individual supervision with their supervisor.
- Provide case coordination, crisis services, supportive counseling and/or assistance when needed per the consumer’s treatment plan.
- Provide backup Intake Service Coordinator duties for referrals, assessments, and admissions to the ACT Unit and New Beginnings when needed.
- Serve on committees as assigned.
- Attends all mandated trainings related to profession and enhances knowledge and/or skills which includes Essential Learning Trainings.
- Supports and helps in the training of new employees.
- Attend scheduled staff meetings as required.
- Transports consumer’s to as needed.
- Cover unfilled Residential Assistant duties and shifts to meet 24-hour residential scheduling needs.
- OTHER DUTIES AS ASSIGNED.
3) Conduct
- Uses clear communication skills with consumers, co-workers and other professionals including using “people first” language both orally and in writing.
- It is expected that clinical staff will seek supervision/ consultation at both scheduled and unscheduled times. For employee’s whose license requires specific supervision, employee has met requirements for clinical supervision based on licensure definitions.
- Maintain accurate and current program records and reports.
- Adheres to Valley’s Standards of Conduct and Compliance in performing services.
- Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the agency, and maintains a good rapport with agency contacts.
MINIMUM QUALIFICATIONS:
- Ability to perform Essential Duties as outlined below.
- BA/BS in Human Service Field.
- Six months to one-year experience with individuals diagnosed with substance use disorder
- Knowledge of treatment resources including a working knowledge of the 12-Step Fellowships.
- Ability to write S.M.A.R.T. goals and objectives.
- Knowledge of the role of the peers, family, and community in recovery.
- Ability to respond professionally and effectively to crises.
- Ability to demonstrate appropriate interpersonal skills.
- Ability to comply with Consumer’s Rights.
- Ability to comply with Valley’s and Division departmental safety procedures.
- Ability to read, write and speak the English language.
- Current Photo ID or Driver’s License.
- Current CPR/AED/First Aid Certificate or ability to be trained.
