We are seeking a compassionate and dedicated Supported Living Specialist to provide personalized support and care to individuals with developmental disabilities. The ideal candidate will possess a strong background in assisted living, home and community care, and patient monitoring, with a passion for empowering individuals to live as independently as possible. This paid position offers an opportunity to make a meaningful difference in the lives of others through direct support and compassionate caregiving.

  • Assist supported living individuals with the following:
  • a. Tenant responsibilities
  • b. Vocational/education interests
  • c. Budgeting/managing personal resources
  • d. Ensuring bills are paid
  • e. Mobility and transportation (with use of agency vehicle) to and from appointments/scheduled procedures, outings, meetings
  • f. Provides transportation (with use of agency vehicle) to access the community
  • g. Manage/participate in the management of his/her medical care including scheduling and attending appointments
  • h. Grocery shopping and meal planning
  • i. Community integration and activities
  • j. Job searching
  • k. Filling out applications/documents for activities, insurance changes, changes in address or other information
  • l. Apartment/household maintenance
  • m. Promoting a healthy lifestyle/physical wellness
  • n. Providing independent living guidance
  • o. Decision making
  • p. Communicating with family, friends, advocates, and the local community
  • q. Promoting independence
  • 2. Reports and communicates information directly to the Director of Residential Services and Program Specialist when necessary
  • 3. Implements individual programs by adhering to daily individual schedule
  • 4. Attends ISP Meetings
  • 5. Implements the ISP
  • 6. Follows written goal plans
  • 7. Records observations of goals in Therap
  • 8. Assisting with scheduling vehicle maintenance of agency vehicles
  • 9. Attends required scheduled staff meetings
  • 10. Participates in trainings, including ISP trainings and others as needed
  • 11. Completes Relias Trainings on shift
  • 12. Attends required in-service training programs (CPR/First Aid & Safety Mechanics)
  • a. This employee is expected to be fully aware of Valley Community Policies and Procedures and to perform work in accordance with such policies and procedures under guidance of supervisors and professional staff.
  • Incident Management
  • 1. Communicates pertinent information regarding individual behavior and significant events by reporting both verbally (over the phone) and in writing (Therap).
  • 2. Ensures the safety of the individual who lives in the Supported Living Site when a reportable incident occurs to an emergency on-call person within 24 hours.
  • 3. Practices responsibilities as a mandated reporter.
  • Additional Responsibilities
  • 1. Conducts self in a manner that respects clients and other staff.
  • 2. Is a role-model for the agency’s philosophy and guiding principles as well as administrative policies of Valley Community Services, including:
  • a. Makes a conscientious effort with regard to job attendance (i.e. arrives on time, reliable, gives ample notice for absences, uses proper procedures of call-offs).
  • b. Approaches individual position positively (i.e. attitude toward staff, individuals, cooperates, follows instructions, shows flexibility, assumes responsibility, uses time effectively).
  • c. Accepts comments and criticism in a constructive way (i.e. approaches criticism with a learning attitude, open to new ideas).
  • d. Works cooperatively with other staff as a team member, helps promote a positive atmosphere (i.e. shares ideas and materials, seeks help and guidance when needed, looks for ways to be helpful, and communicates problems to supervisor).
  • e. Understands job duties and responsibilities (i.e. seeks knowledge of job performances, knows what is expected of them, seeks ways to improve skills).
  • f. Follows Agency policies (i.e. submits medical and TB forms on due date, attends to the duties of their position).
  • g. Respects Agency property (i.e. vans, equipment, and facilities).
  • h. Maintains a neat and orderly personal appearance and wears attire appropriate for the duties of their position.
  • i. Behave ethically by understanding ethical behavior and ensuring that own behaviors and the behaviors of others are consistent with these standards and aligns with the job.

  • First Aid/CPR (annually)
  • 2. Fire Safety (annually)
  • 3. Infectious Diseases and Universal Precautions (annually)
  • 4. Safety Mechanics and Crisis Intervention
  • 5. Chapter 51 requirements(annually)
  • 6. Proficient in use of adaptive equipment
  • 7. Medication Administration

Job Types: Full-time, Part-time

Pay: $19.00 per hour
