VCSB is seeking a compassionate, organized, and experienced Program Coordinator to oversee services for individuals with intellectual and/or developmental disabilities (IDD). This role is responsible for supervising staff, ensuring high-quality person-centered care, and maintaining compliance with regulatory standards. Additionally, this person will provide direct support coordination services to a small number of individuals. Summary of Essential Functions
- Ensure compliance with Medicaid waiver requirements, DMAS, DBHDS, and agency policies and procedures
- Conduct routine audits of documentation to ensure accuracy, timeliness, and regulatory compliance
- Monitor service delivery to ensure individuals receive high-quality, person-centered support coordination services
- Supervise, coach, and evaluate Support Coordinators to ensure strong performance and accountability
- Provide training, onboarding, and ongoing professional development to team members
- Analyze trends, identify service gaps, and implement quality improvement initiatives
- Provide direct support coordination services, including person-centered planning and care coordination
- Coordinate intake processes, including DD waiver referrals, transfers, and waitlist engagement
- Collaborate with internal teams, families, and community providers to ensure continuity of care
- Maintain accurate, timely documentation and ensure compliance with HIPAA and confidentiality standards
Minimum Requirements
- Bachelor’s degree in Human Services, Social Work, Psychology, or a related field
- At least 3 years of experience working with individuals with intellectual and developmental disabilities (IDD)
- Experience with Medicaid waiver services and person-centered planning
- Working knowledge of Department of Behavioral Health and Developmental Services (DBHDS) and Department of Medical Assistance Services (DMAS) regulations
- Valid Virginia driver’s license with a zero or positive points balance.
- Demonstrated experience in documentation, compliance, and quality assurance practices
- Strong organizational, problem-solving, and time management skills
- Ability to manage multiple priorities and meet deadlines in a fast-paced environment
- Proficiency with electronic health records (EHR) and Microsoft Office Suite
Preferred Qualifications
- Master’s degree in Social Work, Counseling, or a related field
- Licensed or license-eligible (e.g., LMSW, LCSW, LPC)
- Previous supervisory or leadership experience
- Experience in quality assurance, auditing, or performance improvement within human services
- Knowledge of Virginia DD Waiver programs and Waiver Slot Assignment processes
- Certification or specialized training in person-centered thinking/planning
- Experience working in community-based services or case management/support coordination
- Familiarity with data tracking, reporting, and performance metrics
- Experience supporting individuals with complex medical and behavioral needs
