Role Description The Nurse Practitioner at Valir PACE will provide primary and specialty care for program participants in an interdisciplinary team environment. This full-time, on-site role in Shawnee, OK includes performing comprehensive assessments, diagnosing and managing acute and chronic conditions, and developing individualized care plans. The Nurse Practitioner will prescribe medications, order and interpret diagnostic tests, and coordinate care with physicians, nurses, therapists, social workers, and other team members. Daily responsibilities include conducting clinic visits, participating in care conferences, monitoring changes in condition, and providing education to participants and their caregivers. The role also involves promoting preventive care, supporting transitions of care, and maintaining accurate, timely clinical documentation in compliance with regulatory standards.

Qualifications
- Clinical and geriatric care skills: ability to assess, diagnose, and manage complex medical conditions in older adults; experience with chronic disease management and preventive care.
- Interdisciplinary collaboration and communication skills: capacity to work effectively within a team-based care model, engage with families and caregivers, and communicate treatment plans clearly.
