LPN needed to support both administrative functions and emergent patient visits. This role combines office-based coordination with field support to ensure timely, high-quality patient care.
Travel: Up to 100%
Duties/Responsibilities:
Assists in the assessment, planning, implementation, and evaluation of patient care within the nursing care plan.
Provides nursing care to patients according to established policies and procedures.
Uses proper techniques, procedures, and safety precautions to meet individual needs of patients.
Checks status of medications provided by Hospice, and reorders as needed.
Performs skilled nursing visits as scheduled.
As authorized, prepares for set-up, hangs, monitors, and discontinues intravenous fluids with accuracy according to direction.
Provides direct care to patients, ensuring safety and comfort of the patient.
Performs treatments and/or assists in treatments and procedures with attention to detail, accuracy, patient safety, and comfort.
Takes and records vital signs of the patient in an accurate and precise manner.
Reports and records any changes in condition or behavior of the patient.
Records all care provided, following established guidelines for charting.
Utilizes the nursing process, and is familiar with other duties, recognizing their importance to patient care.
Participates in patient and family teaching and instruction.
Responsible for completion of assignments while maintaining quality work.
Able to adapt to changes in workload or patient assignments without interference to patient care.
Able to perform in an accurate and precise manner in emergency situations when time is of the essence.
Establishes good rapport with patients, families, and visitors, respecting patients rights to privacy and confidentiality of information.
Communicates pertinent information regarding patient to Interdisciplinary Team, using nursing care plan approach.
Provides support to patients and families in a courteous manner with a positive tone.
Independently performs assigned duties, including after-hours calls.
Participates in sharing of knowledge through in-service, staff meetings, and assisting in orientation of new personnel.
Makes sound judgments based on factual information, seeking guidance and direction when necessary, for performance of duties.
Performs routine duties accurately without physical supervision.
Assist in maintaining equipment assigned to patient care, and reports malfunctioning of equipment to appropriate personnel.
Any and all other duties and responsibilities as assigned.
