Job Summary:
The Hospice Director of Clinical Services is responsible for coordination of direct patient care. They serve as a team leader, providing direction and supervision for direct care staff.
Duties/Responsibilities:
Assigns and coordinates patient evaluation processes and treatments.
Evaluates/considers all aspects of patient needs by incorporating data from medical records, and assessing the patients functional status.
Correlates data from evaluations to determine individualized plan of care.
Follows all established policies and procedures for documentation and form completion.
Delegates patient treatment to support personnel according to their expertise/experience, maintaining the responsibility for the patient and treatment programs at all times.
Administers treatment requiring the advanced skills and knowledge of a specialist.
Determines appropriateness of patients response to care plan, and consults with Interdisciplinary Team concerning any modifications necessary.
Accountable for all operational and fiscal activities of the department.
Responsible for ensuring all staff are properly trained, and that all employees meet annual competencies and educational in-services.
Reviews staff compliance with established policies.
Ensures quality care and excellent communication with patients, family, and staff.
Ensures that department is compliant with all state, federal, and Joint Commission regulations, and reviews policies periodically, making changes as needed.
Any and all other duties and responsibilities as assigned.

Education, Licenses, Certifications and Experience:
Current Registered Nurse license.
Bachelors degree In Nursing.
3 years of experience in hospice or a home health agency or facility.
