We are seeking an experienced and dynamic Primary Care Registered Nurse (RN) to join our Patient Aligned Care Team (PACT) in The Dalles, OR. This role involves demonstrating strong leadership, extensive experience, and innovative approaches to managing complex primary care.
As a Primary Care RN, you will be responsible and accountable for all elements of the nursing process while providing and/or supervising direct patient care. You will assess, plan, implement, and evaluate care based on age-specific components.
**Major Duties Include:**
Care Coordination: Assume responsibility for coordinating care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
Medication Administration: Administer medications and procedures according to established policies and guidelines.
Collaborative Care: Influence care outcomes by working collaboratively with interdisciplinary team members.
Leadership: Demonstrate leadership, experience, and creative approaches in managing complex client care.
Patient Panel Management: Manage assigned patient panels, utilize registries, and ensure appropriate evaluation and access for patient panels.
Health Promotion: Provide health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
Policy Development: Collaborate in developing, implementing, evaluating, and revising policies, procedures, and guidelines.
Improvement: Demonstrate broad leadership and performance to enhance the care within the Primary Care Department and throughout the facility.
Feedback: Provide formal or informal constructive feedback to peers for improvement.
Knowledge Sharing: Support colleagues and other nurses through primary care knowledge sharing to ensure safe, quality nursing care.
Professional Development: Foster a supportive environment conducive to the professional development of healthcare professionals.
Evidence-Based Outcomes: Evaluate outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations.
Documented Outcomes: Responsible for achieving and documenting PACT outcomes at the program or service level. Outcomes must be broad and complex and can be demonstrated at any organizational level within the facility.

Pay: Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
