We're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post-acute and long-term care facilities. You'll conduct assessments, evaluate patients for behavioral health and Value-Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field.

What You'll Do
- Conduct psychiatric intakes and psychosocial assessments for
- Evaluate patients for eligibility across behavioral health and Value-Based Care programs; obtain informed consent.
- Educate patients, families, and facility staff on available behavioral health services.
- Coordinate referrals, transitions of care, and handoffs across the care continuum.
- Serve as clinical escalation point for complex cases and conflict resolution.
- Maintain compliant EHR documentation and review Coordinator documentation for accuracy.
- Mentor and train Behavioral Health Coordinators, including onboarding new team members.
- Flag operational and staffing barriers to leadership.
- Support audits, quality reviews, and compliance initiatives.
- Travel across a defined region to support 8–10 facility sites.
- Active LCSW license (or equivalent independent clinical licensure) in good standing.
- Experience with psychiatric intakes, psychosocial assessments, or behavioral health case management.
- Comfort with EHR systems and digital communication tools.
- Experience with post-acute, long-term care, or geriatric populations a plus.
- Reliable transportation and willingness to travel regionally.
