What are the responsibilities and job description for the Physical (Outpatient) Therapist, Home Health position at UW Health?
Work Schedule:
You may be eligible for a $20,000 sign on bonus.
100% FTE, full-time. Day shift. Monday - Friday 8:00 a.m. - 4:30 p.m. Every third Saturday 8:00 a.m. - 4:30 p.m. You will work at the UW Health 2550 Charles St Clinic.
Additional components of compensation may include:
Evening, night, and weekend shift differential

- Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required
- Current license from the State of Illinois. Required
- Basic Life Support/CPR Certification. Required
- Valid State driver's license and proof of liability insurance. Required
- An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
