Work Schedule:
100% FTE, day shift. Monday - Friday; 8:00 AM - 4:30 PM. This is a hybrid role, with occasional on-site requirements at University Hospital in Madison, WI and the Administrative Office Building in Middleton, WI. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist to:
- Develop and launch pharmacy services with collaboration from the pharmacy manager and clinical team, with the goal of enhancing patient access, optimizing medication access, and improving patient outcomes.
- Deliver medication therapy management services to optimize patient outcomes and ensure safe, effective, and cost-efficient drug therapy.
- Conduct telehealth consultations with headache patients referred by primary care providers and neurology specialists.
- Provide guidance and support for medication access and affordability programs to reduce barriers to care.
The ideal candidate for this role would have a Pharmacy Residency focused in ambulatory care.

- Bachelor's Degree in Pharmacy or a PharmD degree Required
- Completion of an ASHP Accredited Residency training program Preferred
- 1 year of clinical patient care and orders management experience Required
- 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
- Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
- Basic Life Support (BLS) within 90 Days Required
- Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
