- Graduate of an accredited Physical Therapy, or Occupational Therapy, with a minimum of a Master’s degree. Required
- At least five years as a Physical, Occupational or Speech Therapist or closely related field in an inpatient setting or equivalent professional experience. Required
- Current license/certification from the State of Illinois in Physical Therapy, Occupational Therapy, or Speech Pathology. Required
