As an Adult Home Care Physical Therapist, you will:
- Perform comprehensive evaluations and assessments 🩺
- Develop individualized treatment plans tailored to each patient’s needs ✍️
- Deliver skilled therapy interventions to improve mobility, strength, and function
- Support patients in remaining in their homes safely and independently
Minimum Qualifications ✅
- Bachelor’s degree or higher in Physical Therapy
- 2 years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
- Home health experience preferred, but not required, we can train the right person!
- Current Vermont Physical Therapist license, or willingness to obtain one
- Make a tangible difference in patients’ lives daily
