Join UVM Home Health & Hospice as a Medical Social Worker (MSW) II and become an integral part of our Hospice interdisciplinary team. In this role, you’ll provide compassionate, expert social work and care management services in a variety of hospice settings—helping patients navigate challenges, achieve health goals, and experience dignity and comfort.

What You’ll Do

  • Conduct comprehensive psychosocial assessments and develop individualized care plans.
  • Provide support and, care coordination for patients and families facing illness, disability, and end-of-life challenges.
  • Collaborate with physicians, nurses, and other team members to ensure patient-centered and culturally sensitive care.
  • Connect patients with community resources and advocate for their needs.
  • Facilitate family meetings as part of hospice care, as appropriate.
  • Provide bereavement visits and referrals to support family.

Required Education & Experience

  • Education: Master’s Degree in Social Work (MSW)
  • Experience: 2 years of relevant experience (healthcare preferred)
  • Licensure: Vermont LMSW required; LICSW preferred.
  • Travel: Valid driver's license and reliable transportation.

Required Knowledge And Special Skills

  • Knowledge of social work and care management standards of practice, insurance structures, Medicare and Medicaid rules and regulations, clinical practice guidelines, and quality issues related to home health or hospice care.
  • Understanding of barriers and resources available for diverse populations.
  • Skilled in crisis intervention for high-risk situations (abuse, neglect, environmental stressors, suicide, guardianship/conservator issues, long-term planning, unsafe home environments).
  • Awareness of gaps in service delivery systems for the population served.
  • Strong facilitation skills and knowledge of team/small group principles and dynamics.
  • Excellent verbal and written communication skills.
  • Demonstrates integrity and builds honest, trustworthy relationships.
  • Skilled in assessing and providing case management services across the continuum of care, including coping with illness, psychosocial challenges, loss, and grief.
  • Cultural competence and self-awareness of personal beliefs, identities, values, and biases.
  • Working knowledge of medical and mental health diagnoses, disabilities, treatment procedures, substance abuse disorders, medications, and medical terminology.
  • Understanding of mandated reporting requirements for suspected abuse and neglect of vulnerable adults and children.
  • For hospice: ability to perform admissions, supervise day-to-day tasks of team members, and provide bereavement support per hospice conditions of participation.
