We are seeking a dependable and detail-oriented Care Coordination Assistant to support our RN and Social Work Case Managers and help ensure patients and families receive the resources and support they need throughout their hospital stay.
Position Overview
The Care Coordination Assistant provides essential administrative and operational support to the Case Management team, assisting with discharge planning activities, communication, documentation, and coordination of post-acute services. This role helps maintain smooth workflows, facilitates timely transitions of care, and supports communication with patients, families, and interdisciplinary teams.
Key Responsibilities
- Provide clerical support to RN and SW Case Managers, including data entry, scheduling, and documentation.
- Manage phone calls, faxes, and electronic communication related to discharge planning and referrals.
- Prepare and distribute patient packets, discharge instructions, and resource materials.
- Assist in arranging post-acute services such as home health, DME, transportation, and follow-up appointments.
- Contact insurance companies or vendors to facilitate authorizations and approvals.
- Track referrals, services, and discharge follow-up tasks to ensure continuity of care.
- Monitor patient lists and alert team members to pending discharges, delays, or coordination needs.
- Communicate with patients and families to gather non-clinical information relevant to discharge planning.
- Provide resource information or general guidance as directed by Case Managers.
- Help schedule family meetings, interpreter services, or community resource referrals.
- Assist with data collection, metric tracking, and report preparation for departmental use.
- Enter relevant patient information into case management software or tracking systems.
- Participate in daily huddles, interdisciplinary rounds, and team meetings as appropriate.
- Support departmental initiatives to reduce avoidable days and promote timely transitions of care.
- Maintain patient confidentiality and comply with HIPAA, hospital policies, and professional standards.
Qualifications
- Education: High School Diploma or equivalent required. Associate degree or coursework in healthcare, social work, or office administration preferred.
- Experience: 1–2 years of clerical, administrative, or patient-facing experience in a healthcare setting preferred. Experience in case management, discharge planning, or care coordination is a plus.
- Skills: Strong organizational, multitasking, and time-management skills. Excellent verbal and written communication skills. Proficiency in Microsoft Office and EMR systems. Ability to interact professionally with patients, families, and clinical staff. Dependable, detail-oriented, and able to work independently in a fast-paced environment.
