Support Coordinator - Hybrid in SALT LAKE COUNTY

Utah Case Management is looking for a compassionate and dedicated support coordinator to join our team! The Support Coordinator provides coordination services for individuals receiving support through the Utah Division of Services for People with Disabilities (DSPD). Responsibilities include assessing client needs, coordinating care and services, communicating with service providers, and helping ensure clients receive appropriate support.

This position offers a flexible, primarily remote work arrangement, with required in-person monthly client visits. The role is well suited for someone who is self-motivated, detail-oriented, and passionate about supporting individuals with intellectual disabilities.

• Flexible Schedule: The initial time commitment is around 15-20 hours a month, with the potential to increase as the caseload grows. Caseload size is based on the Support Coordinator’s availability and preferred workload.

• Compensation: Pay is based on caseload, with a set monthly rate for each client assigned. Paid training is provided. The estimated monthly rate is $125/client. 

• Location and Travel: The Support Coordinator must reside in Utah within the Salt Lake County area and be willing to travel for in-person client visits. Travel outside the assigned county may occasionally be required.

• Bachelor’s degree in human services or a related field and/or an active Registered Nurse (RN) license.

• At least one year of experience working with individuals with intellectual disabilities.

• Experience working with the Utah Division of Services for People with Disabilities (DSPD) preferred.

• Strong interpersonal and communication skills.

• Self-motivated, organized, and detail-oriented.

• A genuine passion for supporting individuals with intellectual disabilities.

• Able to use personal devices (cellphone & computer/laptop) for work purposes.
