Get to Know Your Team: • UT Health East Texas Rehabilitation Center includes a 48-bed inpatient hospital, 14 outpatient therapy clinics, 11 cardiopulmonary rehabilitation clinics and nine Olympic fitness centers, with access to some of the most advanced therapy techniques and care available. Responsibilities • Evaluates and assesses the client based on subjective and objective information obtained through medical record review, formal and informal test administration with appropriate analysis and interpretation of results to develop the plan of care. Includes OASIS assessment at appropriate time points. • Formulates written functional measurable goals, treatments plans, and home programs for clients. Identifies barriers to describe outcomes.
