Responsibilities
USA Health’s Building Safer Communities Collaborative seeks to utilize a public health approach to address youth gun violence, the leading cause of death among children and teens. The program uses a targeted strategy to reach justice-affiliated youth who are at highest risk of victimization and/or gun violence perpetration. To achieve the goal of reducing, and ultimately mitigating gun violence, we propose a comprehensive plan that establishes a safe network of institutions and systems within the community, builds social capital for youth and families, and prioritizes restorative healing and recovery for all involved. This project will serve as a collaborative pilot study in Mobile County to inform future statewide expansion.
Foster community organization and resource partnerships to ensure the program is addressing the barriers related to the Social Determinants of Health (SDOH) of the community members being served; Creates and implements an outreach strategy for recruiting and engaging a diverse Community Health Worker workforce represented of the communities being served in accordance with grant deliverables; Assists the Community Health Worker Manager in the hiring, training, retention and onboarding of Community Health Workers; Cumulate comprehensive client outcomes data from communities identified by the grant; Identify areas for program continuous quality improvement; Train Community Health Workers on identifying, linking, and navigating SDOH resources to justice affiliated youth and their communities; Work closely with USA Health’s Building Safer Communities Collaborative team to train and integrate Community Health Workers on any internal databases, processes, and procedures of value towards the success of the program; Travel as needed for training, meetings, and workshops; Communicate effectively with teammates and program participants; Work well independently and in a team setting; Ability to maintain participant and client confidentiality; Willingness to complete training as required; Regular and prompt attendance; Ability to work schedule as defined and overtime as required; Perform other administrative tasks as needed.
Qualifications
High school diploma or equivalent, two years of related community-based experience, and completion of a C3 aligned Community Health Worker training program or Bachelor’s degree in Health Promotion, Public Health, Social Work or related field of study from an accredited institution accepted by the University of South Alabama and completion of a C3 aligned Community Health Worker training program; Higher education or related healthcare training program may substitute on a year for year basis for the required experience; Knowledge of local community resources is preferred.
