Role Description This is a part-time, on-site role for a Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) located in Louisville, KY in a medical hospital setting. The clinician will provide individual, group therapy, conduct psychosocial assessments, and develop person-centered treatment plans. Day-to-day responsibilities include maintaining timely documentation, coordinating care with psychiatrists, PMHNPs, and other providers, and supporting crisis intervention when needed. The role involves ongoing monitoring of patient progress, adjusting interventions as appropriate, and providing education and resources to patients and families. The clinician will also participate in case discussions and follow practice policies and ethical standards to ensure high-quality, culturally responsive care.
Qualifications
- Demonstrated skills in Social Work and Medical Social Work, including psychosocial assessment and resource coordination.
- Experience with Case Management, care coordination, and collaboration with interdisciplinary teams.
- Strong background in Mental Health and Behavioral Health, including diagnosis, treatment planning, and evidence-based interventions.
- Active LCSW or LPC license in the state of Kentucky, in good standing.
- Excellent communication, documentation, and clinical interviewing skills.
- Ability to work independently, manage a part-time caseload, and maintain professional boundaries.
- Experience in outpatient psychiatry or integrated healthcare settings preferred.
- Master’s degree in Social Work, Counseling, or a related mental health field from an accredited institution.
