Overview

Patient Navigator Coordinator

The Patient Navigation Coordinator provides individualized assistance to patients, families, and caregivers to overcome healthcare system barriers and access resources throughout the cancer care continuum-from prevention to end-of-life care. Under the guidance of Social Workers, this role supports patients with providing resources, education materials, form completion, and health-related screenings while ensuring compliance with all standards and regulations.

The general pay scale for this position at Willamette Valley Cancer Institute and Research Center (WVCI) is $22.00 - $30.24. The actual hiring rate is dependent on many factors, including but not limited to prior work experience, education, job/position responsibilities, location, work performance, etc.

Responsibilities

  • Connects patients with resources and services for practical needs.
  • Communicating with physicians, advanced practice providers, nurses, social workers, and other members of the care team.
  • Transfer/escalate situations when appropriate with accurate information utilizing resources to resolve inquiries within the scope of the department.
  • Travel intermittently to Northern Clinics in Albany and Corvallis for in-person patient support.
  • Document patient interactions and services in the EMR system.
  • Act as a liaison and advocate for patients and caregivers.
  • Participate in process improvement initiatives to enhance patient outcomes and satisfaction.
  • Maintain strict patient confidentiality in compliance with HIPAA and organizational policies.
  • Perform other duties as assigned.

Qualifications

  • High school diploma or GED; advanced education preferred.
  • A minimum of four (4) years of experience in a clinical health care environment is required.
  • Medical Assistant (certified) experience preferred.
  • Health care-related education, planning, management, or similar experience is preferred.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is required to be present at the employee site during regularly scheduled business hours and regularly required to sit or stand and talk or hear. Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. It requires standing and walking for extensive periods of time. Occasionally lifts and carry items weighing up to 40 lbs. Requires corrected vision and hearing to normal range.

WORK ENVIRONMENT:

The work environment may include exposure to communicable diseases, toxic substances, ionizing radiation, medical preparations and other conditions common to an oncology/hematology clinic environment. Work will involve in-person interaction with co-workers and management and/or clients. Work may require minimal travel by automobile to office sites.
