Social Services Director – Highlands Rehabilitation and Wellness Center
Full-Time | Monday–Friday | No Weekends!

Highlands Rehabilitation and Wellness Center, a 130-bed skilled nursing and long-term care facility, is looking for an experienced Social Services Director to join our leadership team.

What You’ll Do as Social Services Director
- Complete pre-admission screenings, including mental health evaluations per facility criteria
- Conduct and update comprehensive social services assessments
- Serve as Care Plan Coordinator as assigned
- Collaborate with the interdisciplinary team to develop and evaluate individualized care plans
- Protect and promote resident rights and psychosocial well-being
- Document resident progress notes, observations, and interventions
- Manage vendors, specialists, and supportive services visiting the facility
- Support residents and families through admissions, discharges, and transitions of care
- Perform additional duties as needed to support overall resident care and facility operations
What We’re Looking For
- Bachelor’s degree required
- Minimum 2 years of experience in long-term care or a related healthcare setting
- Strong knowledge of state, federal, and Department of Health regulations
- Experience with the survey process highly preferred
- Excellent communication and teamwork skills
- Compassionate, resident-focused, and dedicated to quality care
