Our Skilled Nursing and Rehab facility is seeking a dedicated and experienced Social Services Director. As a Social Services Director you will provide medically related social services that assist the residents to attain or maintain the highest practicable physical, mental, and social well-being. The Social Services Coordinator will manage and oversee the specialists and vendors the visit the facility.
Social Services Director Key Duties
· Conducts pre-admission screening of individuals seeking admission to the facility, including persons with mental illness according to facility admission criteria.
· Completes and updates social services assessments for residents.
· Function as Care Plan Coordinator as assigned.
· Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care to protect and promote residents’ rights.
· Records progress notes in the clinical record. Observations of resident’s behavior and interventions
· Perform other job-related duties as they become required.
Social Services Director Qualifications
- Bachelor’s degree required.
- Minimum of 2 years’ experience in long-term care or a related setting.
- Knowledge of local, state, and federal regulations and Dept. of Health Survey process.
- Ability to work effectively as part of a multidisciplinary team.
