- Location: Saginaw, MI Onsite at skilled nursing & long-term care facilities
- Pay: $123,000-$135,000 base corporate bonus
- Schedule: Mon-Fri, days No on-call No weekends
- Team: Skilled nursing & long-term care partners
- Provide care in nursing and long-term care facilities
- Manage chronic conditions and perform full assessments
- Close care gaps (HEDIS and STAR measures)
- Lead care planning with facility teams
- Support and educate patients' families
- Active, unrestricted APN license in Michigan
- Board certified and Medicare credentialed
- 6 months in skilled nursing or long-term care
- Solid with digital tools and team-based care
- Able to work onsite in Saginaw, MI
- Pay: $123,000-$135,000 base corporate bonus
- Insurance: 90% medical/dental/vision (70% for dependents)
- Other: $10K life insurance, HSA/FSA, 401(k) match
- Time off: Up to 160 hours PTO 10 holidays
- Growth: Paid CE days, stipend, full learning tools
