Background:

Founded in 2004, the mission of Urban Upbound is to provide residents of public housing neighborhoods the tools and resources needed to achieve economic mobility and self-sufficiency, and to break cycles of poverty.

Urban Upbound advances its mission through five integrated programs: employment services, financial counseling, youth development/college and career access, the Urban Upbound Federal Credit Union, and community revitalization services.

Jobs-Plus is a proven place-based employment program designed to increase the earnings and employment of working age public housing residents. Jobs-Plus uses a three pronged approach which includes: employment-related services; financial incentives that make work pay; and community supports for work, the strengthening of social ties among residents and community stakeholders to support residents’ job preparation and work efforts.

Position Description:

Reporting to the Assistant Director, the Clinical and Wellness Coordinator is a key member of the Jobs-Plus team, leading integration of the Connections to Care (C2C) initiative into the Jobs-Plus program. The Clinical and Wellness Coordinator will coordinate staff trainings with the Mental Health Partner, provide coaching and supervision of staff related to the mental health needs of members, participate in case conferences, and coordinate pathways to clinical care for members.

Responsibilities:

• Creating and implementing strategies to increase outcomes in the following categories:
• Communicate with members’ health care providers; returning telephone calls to clients and other medical and community health care agencies providing services to clients
• Oversee technical assistance engagement of the site, participating in Case Conferences, and coordinating pathways to clinical care for members
• Conduct “Wellness” education groups and one on one sessions
• Assist with intakes, identify clients’ current medical status, follow up on what has been reported and indicated in supporting documents
• Conduct and review PHQ9’s
• Collaborate with ThriveNYC programs and support centers, referring members to screenings and vaccination services at local, free and affordable hospitals, and other CBO providers, and education providers
• Develop and maintain referral partnerships with mental health and wellness providers
• Partner with Citibike initiative to provide new Citi Bike membership in low ridership developments for free to balance a healthy lifestyle
• Develop and facilitate mental health and wellness events to engage and teach the community about living a healthy lifestyle

Other Responsibilities include:

• Develop customized care plans for clients including attainable goals and plans of action
• Ensure the integrity of data entered in the program database and client case files
• Analyze data to inform and make recommendations and referrals to appropriate resources
• Other duties as assigned by leadership

Qualifications:

• Must have a Bachelor’s degree; Masters Degree in clinical psychology, social work or public health, or similar preferred.

• Minimum of five (5) years of experience working in social services, mental health and/or substance abuse, or related fields
• Strong ability to implement community mental service models
• Adheres to all Human Resources policies, procedures, and codes of conduct
• Computer proficiency in Microsoft Office, Google Docs, Internet Research, and social media
• Outcomes-driven, strong analytical skills with experience using data to inform programmatic decisions
• Highly motivated, with proven capacity to work independently, take initiative
• Flexible, with availability to work a weeknight and occasional Saturdays
• Navigate and negotiate through difficult situations

Pay: $50,000.00 - $55,000.00 per year
