Job Title: Case Manager

Location: Bronx, NY
Reports To: Director of Social Services
Salary: $50,000
Job Summary:
Urban Rebound Initiatives (URI) Inc., is seeking Case Manager (CM) to provide assistance to asylum seeking clients within a Bronx facility. The CM is to ensure effective and adequate support is provided in a timely manner to the clients as well as providing referrals to community resources to address immediate and long term needs.
Duties/ Responsibilities:
- Conduct intake of new clients
- Assess well-being of clients
- Orient new clients to Department of Homeless Services policies, procedures, and programs
- Maintain accurate records for each client
- Develop Individual Living Plans (ILP) with clients
- Document interactions and correspondences in relation to clients
- Assist clients with obtaining identifying documentation
- Provide community resource referrals for current and future needs
- Assist clients in navigation of government and non-government systems
Required Skills/ Abilities:
- Knowledge of CARES preferred
- Must be proficient in computer system navigation
- Cultural competency a must
- Bilingual/ Multilingual is a plus
Education and Requirements:
- Bachelor’s Degree; or
- Case management experience
Case Managers report to Director of Social Services. Director of Social Services reports to Program Director.
Job Type: Full-time
Pay: $50,000.00 per year
